Providers who are interested in participating in our preferred network to perform IME’s, peer reviews, or medical imaging services are invited to send an inquiry to us.  We will send you an application, questionnaire, contract, and our request for credentialing documents.

Requirements to become a Med-Eval preferred provider:

• Must be in private practice.
• Must be board certified in your medical specialty.
• Must complete and return our application, questionnaire, contract, and our request for credentialing documents.
• Must be approved through our credentialing process.
